Clinical Application of Octreotide in Postoperative Enterocutaneous Fistulas



Abstract
The long-acting somatostatin analogue, octreotide, is a synthetic cyclic peptide consisting of 8 amino acids and is known to be effective in the conservative treatment of enterocutaneous fistulas.
We conducted retrospective review of 30 patients with postoperative enterocutaneous fistulas to see therapeutic effect of octreotide. To examine the therapeutic effect of octreotide on the enterocutaneous fistulas, 30 postoperative complicated
cases
were analyzed into two groups: Group A(15 cases); treated with general supportive cares including TPN(total parenteral nutrition) as a control, and Group B(15 cases); treated with general supportive cares, TPN and octreotide.
In group A, the daily output of fistula dereased to less than 50% in 11 days. 5 cases have faild to closure, but 10 patients could achieve fistula closure in 26.9¡¾11.4 days after TPN. In other hand, group B showed decreasing fistula amount to

in 4
days after octreotide. 2 cases have failed to closure, but 13 patients have resolved the fistula in 8.2¡¾1.4 days after octreotide, as well as in 13.9¡¾3.7 days after TPN. The difference of the treatment results of each group was significant.
With these results, we conclude that the conservative treatment combined with octreotide is effective in the postoperative enterocutaneous fistulas. Prospective, disease-matched, randomized trials would be required for the future.
